Odontogenic keratocysts (OK) are benign odontogenic cysts known for their aggressive behavior and high recurrence rate. A 39-year-old female patient with a history of breast cancer six years ago, without bone metastases or antiresorptive medication use, was referred to the oral surgery service due to a radiographic finding. The clinical examination showed no intraoral or extraoral changes. Imaging revealed a well-defined unilocular radiolucent area distal to tooth 37. An incisional biopsy confirmed the diagnosis of OK. Treatment included enucleation, curettage, and chemical cauterization with 5-fluorouracil, which was retained in the cavity with gauze and removed after 24 hours. The patient has been under radiographic follow-up for 12 months without recurrence. The complementary use of 5-fluorouracil after lesion removal appears effective in reducing the recurrence of OK.
