Oral lichen planus (OLP) is an autoimmune condition that affects the squamous epithelium of the oral mucosa and is also recognized as a potentially malignant disorder. OLP predominantly impacts female patients and is seldom observed in adolescents and young adults. Here, we present a case of a 20-year-old patient diagnosed with OLP. During the physical examination, the most noteworthy findings included hyperpigmented lesions on the lower lip and the classic presentation of reticular OLP, marked by bilateral symmetrical striae on the buccal mucosa. Considering the clinical suspicion of OLP, an incisional biopsy was conducted. Microscopically, the examination revealed areas of atrophic epithelium with saw-tooth rete pegs, liquefactive degeneration of the basal cells, presence of Civatte bodies, and a dense band-like lymphocytic infiltrate in the connective tissue. The final diagnosis was confirmed as OLP. As of the last follow-up, which was approximately 10 months ago, the patient had not been prescribed any treatment as they showed no symptoms.
