A typical squamous cell carcinoma of the lip typically manifests as a firm, painless, crusted, exuding ulcer, generally small and measuring less than 1 cm in diameter. It exhibits slow growth and is histologically well-differentiated. However, we are reporting a series of 5 cases of lip SCC with atypical clinical or histological presentations. The series comprised 4 males and 1 female, with an average age of 54.6 years (ranging from 44 to 81 years). In the first case, the presentation was that of a deep necrotic ulcer, and histologically it was poorly differentiated. The second case featured a large, pedunculated nodular lesion. The third case, apart from being located on the upper lip, was histologically diagnosed as basaloid SCC. The fourth case presented as a 4 cm diameter ulcer with raised edges. The fifth case presented as a crusted ulcer that extended to the skin of the chin. All patients were referred to a specialized hospital where they received the necessary treatment.
