Abstrakt: Background: Hepatoblastoma (HB) is the most frequent malignant liver tumor in infants and young children. While it is rare, the brain seems to be the most habitual site of non-pulmonary metastasis. Case presentation: In this report, we present the case of a 13-month-old girl followed for metastatic hepatoblastoma in the lung. She was brought to the emergency department with tonic–clonic seizure, and a brain CT scan revealed a single intra-parenchymal metastasis. Conclusions: In children with a history of hepatoblastoma, the development of neurologic symptoms should prompt cerebral imaging, to exclude the presence of cerebral metastases.
