The use of dexmedetomidine as an alternative pre-medication in children with autism attending elective day case dental treatment.

Abstrakt: Background Children with autism spectrum disorder (ASD), especially those with oppositional behaviour may require dental treatment under general anaesthesia as routine dental treatment is difficult to provide. Pre-medication is often needed to facilitate anaesthetic induction. Aim The aim of this study was to evaluate the feasibility and tolerance of dexmedetomidine as a pre-medication for paediatric patients with ASD in a high turnover day case setting. Method This two-part study involved a retrospective evaluation of records of patients with ASD, who attended for dental treatment under general anaesthesia in Guy's Hospital Day Surgery Unit or Evelina London Children's Hospital, and received pre-medication with dexmedetomidine. The second part of this study involved an online questionnaire given to carers to assess satisfaction. Results Sixteen patients with ASD were included in this study. No adverse events were recorded. Two patients required post-operative opioids. There was a minor increase to discharge time, possibly due to the sedative nature of dexmedetomidine, or staffing concerns regarding use of a new medication. Mostly positive feedback was reported via the online questionnaire. Conclusion This feasibility study suggests that dexmedetomidine can be used as a pre-medication for children with ASD who are referred for day case dental procedures under general anaesthesia.
