A 35-year-old White Brazilian female patient was referred for assessment of a rapidly enlarging asymptomatic lesion in the maxilla, present for roughly 4 months. Extraoral exam revealed facial asymmetry caused by enlargement of the facial lower third. Intraoral examination showed a firm erythematous tumor at the right upper alveolar ridge extending to the right upper vestibule and palate. Imaging demonstrated a poorly defined, slightly radiopaque/hyperdense maxillary destructive lesion invading the right maxillary sinus. The diagnostic hypotheses were of benign odontogenic neoplasia or malignant mesenchymal neoplasia. An incisional biopsy was performed and a diagnosis of ossifying fibroma was made. The patient was submitted to local excision and the final diagnosis was benign a fibro-osseous lesion. The patient is on ongoing clinical monitoring and there are currently no signs of recurrence.
