In recent years, the literature presented a clinical variant of bisphosphonate-related osteonecrosis of the jaw (BRONJ), represented by the absence of bone exposure in the mouth, resulting in a discussion about the clinical classification proposed previously. The aim of this study was to present a clinical case of non-exposed BRONJ. Few studies about non-exposed BRONJ were observed in the literature. The clinical staging classification should be improved. The dentist must be aware of the medical history of the patient and, imaging exams should be carefully analyzed in order to find initial bone alterations and to prevent future complications.
