Introduction: Melioidosis is endemic in Southeast-Asia, including Malaysia. The infection is commonly acquired via contact with infected soil, inhalation, aspiration, ingestion and percutaneous inoculation. Objective: To highlight a case of septicaemic melioidosis, and discuss the literature. Method: We report a case of septicaemic melioidosis, where the deceased was brought in dead to the mortuary. Result: Post-mortem examination showed congested and oedematous lungs with multiple micro-abscesses mimicking military tuberculosis. Burkholderia pseudomallei was detected from the blood culture, spleen and lungs tissues. Conclusion: This case highlights the necessity to include melioidosis as a differential diagnosis in investigating respiratory infection presented as sudden death, especially in the region, where melioidosis is endemic.
