Extending the use of peripheral intravenous catheter and administration sets from 72 hours to 96 hours.

Abstrakt: Aim: The aim of this study was to examine the incidence of local catheter-related infection, catheter-related bloodstream infection and infusate related bloodstream infection when peripheral catheters and administration sets are left in place up to 96 hours.Methods: A prospective observational study was done. After the recruitment of subjects at 72 hours by the infection control nurse, patients who showed no signs of phlebitis had their IV catheters extended to 96 hours. The catheter sites were monitored by ward nurses according to a predetermined definition for phlebitis. When signs of phlebitis were present, ward nurses sent the catheter tips and infusates to the laboratory for culture. The infection control nurse correlated the culture results and the catheter site assessment to detect any catheter-related bloodstream infection or infusate-related bloodstream infection.Results: Two hundred and twelve samples were recruited in the 16 months surveillance. Five samples of 166 (samples with IV peripheral lines and administration sets for 3 days) exhibited presence of phlebitis. Their cultures showed no significant bacterial growth. The phlebitis rate was 3%. The researcher found that the infection rate of prolonging IV placement to 96 hour had a confidence interval of 0% to 1.8%.Discussion: The research has not demonstrated any local catheter-related infection, catheter-related bloodstream infection and infusate related bloodstream infection. A proposed randomised control study could be adopted to recruit more samples and reduce bias in future study.Conclusion: The results supported the recommendation by the Centers for Disease Control and Prevention and a recommendation to the Hospital Infection Control Committee to replace the peripheral intravenous catheters and administrations set at 96 hours will be made.
