Melena defined as black as tar dirt because of the blood that is changing shape. Bleeding can occur from the esophagus, stomach or duodenum. But lesions in the jejunum, ileumevenasendes colon can cause melena provided that the travel time through the astrointestinal tract long enough. Approximately 60 mL is sufficient to cause a bowel movements with black stools. Acute blood loss greater than this amount can cause melena more than 7 days. Patient Mr. M aged of 67 years entered RSAL Dr. Mintohardjo September 5, 2014 with a diagnosis of Melena. Therapy treatment for 8 days, PRC transfusion (Packed Red Cells), infusion of Ringer's lactate, Transamin (tranexamic acid), Vitamin K, Sucralfate, omeprazole, Dexanta (aluminum hydroxide, magnesium hydroxide, simethicone), Sanprima F (Sulfametoxazole, trimethoprim) and ketoconazole.
