Candida endocarditis is a rare infection that for many years has been associated with injection drug users in the community setting. Over the last 20 years, there has been a shift in the epidemiology of Candida endocarditis from community onset towards a healthcare-associated disease. Major risk factors now identified are prior infective endocarditis, immunosuppression, prolonged use of intravenous antibiotics, central venous catheters, and prosthetic heart valves. The increased use of cardiac rhythm management devices and ventricular assist devices has become a new risk factor for the development of healthcare-associated Candida endocarditis. Of concern is the emergence of Candida endocarditis in the pediatric population, especially amongst neonates with prolonged hospital stays and/or intracardiac defects. Whether seeing community or healthcare associated Candida endocarditis, mortality from this disease remains unacceptably high, and suspicion for infection in patients with risk factor warrants early diagnosis.
