The article discusses various aspects of advance care planning which is defined as the process of formal decision making aimed at helping patients establish decisions about future medical care that are followed when they lose the capacity to make decisions. Background is presented on the history of adoption of advanced care planning in medical care. The benefits of advance care planning are discussed including less aggressive medical care and better quality of life near death.
