Panniculectomy in Gynecologic Surgery.

Abstrakt: The goal was to evaluate the utility and morbidity of panniculectomy in gynecologic surgery. A retrospective chart review was conducted for 10 morbidly obese patients who had large abdominal panniculectomy with abdominoplasty at the Department of Obstetrics and Gynecology, University of Bari, Italy, between 1995 and 2001 in order to allow a better exposure of pelvis and abdomen and thus facilitate surgery. The mean age of the patients was 48 years (range, 39-63), mean weight was 111 kg (range, 94-154) and mean body-mass index (BMI) was 41 kg/m[sup 2] (range, 31-49). The most common comorbidities in this population were hypertension and diabetes. All patients underwent total abdominal hysterectomy with bilateral salpingo-oophorectomy; 5 patients had also pelvic lymphadenectomy. Median duration of surgery was 350 minutes (range, 280-450), average estimated blood loss was 700 mL (range, 35-1400), and 5 patients received transfusions. Median weight of the panniculectomy specimens was 2300 g (range, 1650-3100). Postoperative complications included ileus in 3 cases and superficial skin-subcutaneous wound necrosis in 5 cases. No patient was operated on again. This study supports the safety of panniculectomy in gynecologic surgery. [ABSTRACT FROM AUTHOR]
