Abstrakt: SUMMARY 1. The movement patterns of the normal pelvi-ureteric segment as seen on cinepyelography, at operation or during perfusion, suggest that co-ordinated contractions of the renal pelvis and upper ureter are associated with a uniform distribution of mixed muscle spirals at the pelviureteric junction. 2. In congenital hydronephrosis pelvi-ureteric inco-ordination can be demonstrated at operation or during perfusion by raising the intrapelvic pressure, and may be associated with a predominance of longitudinal muscle fibres at the pelvi-ureteric junction in place of the normal spirals. 3. The part played by this defect in the genesis of the various clinical types of hydronephrosis is outlined and a possible mechanism for the production of the pelvic dilatation is suggested.
