Abstrakt: A questionnaire is a method for collecting data that can come from many different sources: from observations, telephone interviews or documentary sources. Whatever the source of data is, the questionnaire provides a framework of questions that facilitate researcher's work. A manual approach for collecting data using questionnaire, presents some limitations and introduces several sources of errors. An Informatic System was implemented to reduce these errors and to support researchers in epidemiological studies. For the experimentation of the prototype, a paper format questionnaire has been digitalized from the original sheet provided by the Chair of Hygiene of the Magna Graecia University of Catanzaro. The implemented system allows researchers to create questionnaires, adding sections and structured questions. The administrator of the system can visualize a preview of the questionnaire and decide which group of users can compile it. The system provides a control panel to analyze the collected data and that permits the exportation of saved data into statistical software compatible formats. [ABSTRACT FROM PUBLISHER]
