Left ventricular (diastolic) dimensions and relaxation in patients with atrial fibrillation*.

Abstrakt: In patients with atrial fibrillation the relationship between varying RR-intervals and left ventricular (diastolic) function parameters was studied. M-Mode echocardiograms showing left ventricular internal dimensions were continuously recorded during about 100 heart beats from nine patients. Left ventricular end-diastolic dimension (LVEDD), left ventricular end-systolic dimension (LVESD) and a number of derivatives were obtained and used as contraction- and relaxation-parameters.The results demonstrate that left ventricular contraction is related to preceding and pre-preceding RR-intervals, but that no relation could be demonstrated between left ventricular relaxation and preceeding RR-intervals.In patients without mitral stenosis a biphasic relation was found between left ventricular diastolic dimension and preceding RR-intervals. consisting of(1) a positive almost linear part in early diastole, followed by(2) a plateau phase in late diastole.This observation points to early diastolic ventricular filling which reaches its maximum well before the end of the diastolic pause.
