Morphological defects in the dental structure such as a palatal groove can be predisposing factors for the onset of inflammatory processes in the periodontal and/or pulp tissues. It is critical to recognize these, because of their diagnostic complexity and the problems that may arise if they are not correctly interpreted and treated. Careful use of endodontic and periodontal surgical procedures may restore satisfactory function. In the present case, the patient presented with a deep probing defect and advanced bone loss. Root canal therapy was performed followed by flap reflection, removal of granulation tissue and sealing the groove with conventional glass ionomer cement. Bone graft was placed followed by a resorbable barrier for guided tissue regeneration. Their use in the treatment of palatal groove showed an improvement in clinical attachment, reduced probing depth and deposition of bone in the osseous defect. A 24 month follow-up is included.
