Necrosis and infection are common in malignancies in part due to rapid growth. Uterine leiomyosarcoma is not an exception. We report a case of a postmenopausal woman with a history of abnormal uterine bleeding who did not seek medical attention for 2 years presented to the emergency room complaining of fever, abdominal pain and purulent vaginal discharge. A computed tomography revealed uterine mass with signs of infection and presence of multiple lung metastases. Parenteral antibiotics were administered before exploratory laparotomy with total abdominal hysterectomy. After 6 months of the surgery and two cycles of chemotherapy, disease was in remission.
