Three cases of anteroseptal myocardial infarction are presented in whom the development of left anterior hemiblock (LAH) led to the replacement of the characteristic Q waves by initial rwaves in the precordial leads. This change attains added significance, particularly in the out-patient population, as it may obscure the electrocardiographic pattern of old anteroseptal myocardial infarction. Factors that influence the occurrence of the initial rwaves in the right precordial leads were discussed. Vectorcardiography may be necessary to confirm or disprove double pathologies in some cases. We have highligted the significance of scrutinizing the precordial leads in addition to the limb leads when evaluating patients with coronary heart disease and LAH. [ABSTRACT FROM AUTHOR] |
