For long years, so many controversies on the cancerous change from the gastric ulcer have been discussed but have not been able to get a decisive conclusion, because they were studied chiefly on surgically resected stomachs, which were only a phase of long course of the gastric cancer. We have tried to clear this problem, from the standpoint of clinical follow-up studies for many years with fiberscopic biopsy and gastrocamera pictures. Endoscopie follow-up studies are classified into two methods: prospective and retrospective studies. Under a prospective study, it is denned as a follow-up study on benign lesions with fiberscopic biopsy, until some of them might be detected their malignancy. Out of 239 cases of prospective studies on the benign gastric ulcer for more than two years, 5 cases (2.1%) of the gastric cancer could be detected by repeated and periodic biopsy. A retrospective study is a method collecting past gastrocamera films of proven malignant lesions after surgery which were thought as benign lesions in their courses. According to this method, out of 43 cases observed ulcerous lesion in their courses, in 5 cases of the gastric cancer, ulcerous lesions were recognized at the same place of the carcinoma, before 10 years. These cases may be thought as ulcer-preceding cases or malignantly changed cases from the benign gastric ulcerous lesion. We suppose that real frequency of the cancerous change from the gastric ulcer, if possible, may be fewer than these ratios, because another greater number of the gastric ulcer are not used to follow up with periodic biopsy for more than two years.
