The increasing incidence of type 1 diabetes in North Wales 1966-2002.

Abstrakt: The incidence of type 1 diabetes has increased over time but the factors responsible remain uncertain. Using prospective and retrospective data we aimed to model changes in incidence rate from 1966-2002 and to assess change in inherited factors over time (the gene pool hypothesis) from family history data. Data from multiple sources were used to calculate patient numbers. Retrospective data were collected for those with onset before 1990. Information on deaths was obtained from the Office of National Statistics. Family history data obtained by questionnaire were used as a marker of inherited influences. The study was conducted in Clwyd, North Wales, and included all patients with type 1 diabetes diagnosed before age 15 and resident in the catchment area in March 1998. The results showed that measures of case ascertainment were satisfactory. The incidence of type 1 diabetes before age 15 increased from 12.2 (95% confidence interval 9.2-16.1) in 1966-70 to 31.3 (23.1-41.3) cases per 100 000 population per year in 2001-02. Examined over the whole period 1966-2002, the increasing incidence before age five was not different from that in older children. There has been no change in the number of patients with a family history of type 1 diabetes per 100 000 children nor in the proportion of type 1 diabetic children with such a family history. In conclusion, the numbers of children developing type 1 diabetes continue to increase.
