Abstrakt: A complete purpose-designed diabetes record system was introduced for the hospital-based diabetes service at Stoke Mandeville in 1982. It seeks to overcome many of the deficiencies inherent in hospital records and has proved effective, easy to use, and inexpensive. Even in its original form it has been flexible enough to accommodate new ideas and alterations in working practice, but further refinements were made for its second edition (January 1988). The record system itself has provided a valuable infrastructure for coordinating the activities of the multidisciplinary team working in the continually changing situation of our developing District Diabetes Centre ( Ref 1).
