An 80-year old woman presented with spontaneous bruising and anaemia which proved to be a manifestation of consumption coagulopathy secondary to a dissecting thoracic aortic aneurysm. The history of dissection was classical and her aorta was hugely dilated. She had recovered spontaneously after two weeks bed rest and had no symptoms until haematuria and bruising occurred. Treatment with a variety of regimes was tried. No single therapeutic strategy was successful and she required repeated blood transfusions, platelet and plasma infusions for a period of 18 months before she died suddenly. It is recognized that acute haemorrhagic diathesis may complicate the early days after acute aortic dissection. This case is unusual by virtue of the chronicity of the haemorrhagic tendency, and its mode of presentation. [ABSTRACT FROM PUBLISHER] |
