Patellotibial external fixation for ruptures of the distal extensor apparatus of the knee.

Abstrakt: The Marburg external patellotibial (Mpt) fixator is designed to temporarily protect a reconstructed distal extensor apparatus of the knee. It allows an early functional aftercare. Acute and chronic disruptions of the distal extensor apparatus. Severe metabolic diseases. Previous bone and/or soft tissue infection. Uncooperative patient. Assembly A: insertion of a threaded pin into the patella perpendicular to its long axis. Reconstruction of the extensor apparatus. Insertion of a second threaded pin 0.5 cm distal to the anterior tuberosity through the tibia in the frontal plane. Attachment of the connecting rods. Assembly B: same as assembly A but for the second pin which is inserted in a sagittal direction. Since 1990 the Marburg external patellotibial fixator was used in 21 patients (4 women, 17 men; average age 39 years). A proximal avulsion of the patellar ligament or a comminuted fracture of the lower patellar pole was present in 12 patients (group I) and a mid substance tear of the patellar ligament in 9 (group II). Follow-up after an average of 42.3 months was possible in 19 patients. Based on the score of the International Knee Documentation Committee (IKDC), 3 patients had a normal, 12 a nearly normal, 4 an abnormal, and none a severely abnormal function.
