Introduction. Trunk asymmetry in normal children becomes evident in juvenility, earlier in girls than boys. It is likely that AIS in girls is also initiated in juvenility. Objectives. To examine whether there are skeletal growth differences between normal juvenile girls and boys. Materials and Methods. Normal children age 5–10 years were measured using Holtain equipment for skeletal lengths by one observer (girls 172, boys 178). The data are analysed by higher (H) and lower (L) BMI subsets relative to median BMI values for age and sex. Results. Mean BMI values for girls and boys respectively H/L are: 17.32/17.38 and 14.62/14.65, neither group statistically significant, suggesting similar circulating leptin levels in girls andboys. In H girls, skeletal segments are significantly shorter than boys' for 7/13 skeletalparameters, and not in L girls, with p values as follows (right limbs), respectively H/L: standing height p=0.066/0.982; sitting height 0.012/ 0.712, subischial height (SIH) 0.343/0.729, biiliac width 0.604/0.487, biacromial width 0.022/ 0.767, upper arm length 0.029/0.423, forearm-with-hand-length 0.003/0.375, tibial length 0.748/0.347, foot 0.005/0.636 (ANOVA correcting for age). Conclusion. Subset H girls produce less segmental skeletal growth for age than boys except for pelvis, SIH and tibiae. Growth-plate mechanisms that determine this sexual dimorphism may involve intrinsic receptor responses to hormones and/or central controls, for which we suggest centralleptin resistance (CLR) in the somatotropic axis. Significance. Such somatotropic CLR could limit the energy invested in female juvenile skeletal growthconserving energy for reproductive development and predispose girls more than boys to AIS.
