Can we interpret the orientation of major or minor orifice in implanted tilting disc mechanical mitral valve on X-ray chest P-A view ? “The Sion Sign”.

Abstrakt: Chest X-ray is an important investigation in a patient Following Mitral valve replacement. After Mitral valve replacement in Chest X-Ray Postero-Anterior (P-A) view the Prosthesis in the mitral Position is seen in sagital open position as opposed to a prosthesis in Aortic Position which is seen as coronal slit. Tilting Disc Mechanical Heart valves have a Major Orifice & a Minor Orifice. This can be very well appreciated in Mitral Position due to special design in housing & strut position. The anatomical verses Non anatomical orientation of a Tilting Disc prosthesis is well known in Aortic as well as in Mitral position. In Mitral position there is a revised interest in orienting the Major Orifice Posteriorly as it helps to achieve physiological flow especially in tilting Disc Mechanical Mitral valve. The Chest X-ray studied following single double or triple valve valve replacement revealed that at least in 95% of the cases it was possible to interpret the orientation of major orifice of Tilting disc in mitral Position with respect to the aortic annulus and ventricular apex. A simple Post operative Chest — Ray can provide valuable information regarding the orientation of the Mechanical tilting disc heart valve in mitral position. [ABSTRACT FROM AUTHOR]
