We report a case of cerebral death in a pregnant woman. In the literature, more than 12 cases have already been published, but this is the first concerning a patient admitted to an Italian hospital. Media reports suggest that other similar cases have occurred in Italy, although they have not been published. This is most likely due to the tendency of reporting only the positive outcome cases with regard to the foetus, not reporting all other cases. We believe that the few cases reported and the possibility of a good outcome despite all the difficulties, make it still useful to compare the clinical, intensive care procedures, obstetrical-gynaecological, legal, ethical and neonatal problems which can present when dealing with such cases.
