The authors reflect on the effectiveness and efficiency of the use of root cause analysis in determining the cause of medical errors in medicine. They suggest that while a structured approach to investigating and mitigating medical hazards and errors is desirable there are problems with root cause analysis. They argue that there is an undercurrent of sentiment in the medical community that the effectiveness of root cause analysis is limited |
