AIMS Continuous endocrine changes during pregnancy results in increased insulin resistance. Gestational Diabetes has the same pathogenic pathway like Metabolic Syndrome. Insulin resistance is the root cause in both. Keeping this in mind if women with PCOD, BOH, Macrosomia IUGR who were not investigated for glucose intolerance during the conception period is followed and if screened scrupulously, Diabetes can be detected early and diabetes as such or any Abnormal Glucose Tolerance (AGT) can be prevented or its complications delayed or prevented in later Life. OBJECTIVES GDM is a double edged weapon affecting both the mother and the offspring. Since the foundation stone for diabetes starts a decade early, early detection and treatment of intrauterine hyperglycemia not only prevents BOH, Macrosoroia IUGR but also decreases the risk of obesity CVS problems and diabetes in the later life of the offspring. So all the middle aged Females with past H/O BOH, Macrosomia IUGR, should be periodically screened for AGT in furture. MATERIALS 200 Female Diabetics who did not have any complications during pregnancy detected clinically were interviewed about their past obstetrical History. They were between the age of 30-45 years and their duration of diabetes 1-20 years. All were primi or multiparas. Last Child Birth (LCB) was from 5-25 years. METHOD All these randomly selected Female Diabetics were questioned thoroughly for their past obstetrical H/O PCOD, BOH, Macrosomia IUGR miscarriages, congenital anomalies, neonatal death, still birth, RDS were all noted clearly. Age, Height, Weight, BMI, FH, parity were all noted. RESULTS Among these 200 Female Diabetics 53 had BOH alone 48 had marosomia alone, 52 had both and 47 of them did not have any of these. Totally 105 had BOH, and 100 had macrosomia 6 had IUGR CONCLUSION Since the above study show that maoirty of the present female diabeties have past H/O some form BOH, Big babies, IUGR etc, Condersely screening all the non diabetic female who have past history as above for presence of diabetes will reveal many new diabetics or IGT which can be either reverted to normal or complications prevented.
