Awareness of peripheral arterial disease (PAD) is low among physicians, and the recommended test for its detection (ankle-brachial index -ABI-) is not usually performed in routine clinical practice. However, PAD is a coronary heart disease (CHD) risk equivalent and a tight control of classical cardiovascular risk factors (CCRF) is recommended. To assess in type 2 diabetes mellitus (T2DM) subjects whether those with PAD have a different control of CCRF in relation to subjects without PAD, 31 spanish diabetology centers consecutively screened for PAD (ABI) 1305 subjects (age:65.4±7.7yr, women:52.2%), recording information on control of CCRF and previously known cardiovascular disease (CVD: CHD or stroke). 292 subjects showed PAD (ABI<0.9). Subjects were classified into 4 groups: 1) without previously known CVD and no PAD (n=790); 2) without previously known CVD and PAD (n= 194); 3) with previously known CVD and no PAD (n=182); 4) with previously known CVD and PAD (n=96). The control of the CCRF was (respectively): Non-smokers: 84.8%, 79.0%, 93.3%, 86.1% (2 vs 3 p<0.05); antiplatelet treatment: 36.1%, 40.2%, 80.2%, 79.0% (2 vs 3 and 2 vs 4 p<0.05); systolic and diastolic blood pressure < 130/80mmHg: 18.0%, 12.8%, 20.3%, 15.6% (2 vs 3 p<0.05); LDL cholesterol <100 mg/dl: 43.8%, 34.4%, 59.9%, 64.7% (2 vs 3 and 2 vs 4 p<0.05); HbA1c<7.0%: 39.1%, 32.8%, 28.5%, 34.1% (NS). We conclude that the detection of PAD by the ABI identifies a group of subjects with a poorer control of CCRF than subjects with other clinical manifestations of CVD, thereby increasing the value of ABI in the routine care of subjects with T2DM.
