Diabetic patients are often threatened by asymptomatic arrhythmia that can cause cardiac sudden death. Self-monitoring of ECG offers a fast way to record ECG according to a set schedule regardless of patient's symptom, and has an important advantage which may help early detection of cardiac events. We developed a portable, compact, non-transmission type ECG self-monitoring system, HCG-801 (OMRON HEALTHCARE, Japan). The purpose of this study was to verify the usefulness of HCG-801 as a device to detect asymptomatic arrhythmia in diabetic patients. Forty five Type 2 diabetic patients (28 males and 17 females aged between 46 and 81, HbA1c 6.6 ±1.0% (mean ±SD)) who had not been diagnosed with arrhythmia were instructed to record ECG for 30 s three times a day (before breakfast, after dinner and bedtime) and after exercise. The average number of recording was 50.8 ± 25.4 during 15.5 ± 4.2 days/person (3.3/day person). The total number of recording was 2284. Ninety six % of ECG waveforms were appropriate for doctor's evaluation. Asymptomatic arrhythmia was detected in 18 subjects (41.3%): premature ventricular contraction (PVC) in 11 subjects including 3 cases of multifocal PVC, and premature atrial contraction (PAC) in 11 subjects including 1 case of solitary PAC and 1 case of atrial bigeminy. Arrhythmia tended to be more frequent in the subjects aged 75 or older (80.0 episodes, 28.6%) (p = 0.07), and with hypertension and/or organic heart diseases (53.6 episodes, 17.6%, p = 0.10). Frequency of PVC detection varied between 1.2 and 52.3% of the total number of recording in each subject, suggesting that PVC can be detected even when its frequency was low. HCG-801, a simple and compact ECG self-recording system, may have a high potential to be proactively used as an effective screening tool of asymptomatic arrhythmia. [ABSTRACT FROM AUTHOR] |
