The author explains why physicians and health care institutions in Canada should support disclosure of harmful errors. He discusses the ethical and practical reasons for diclosing errors and the recent changes that support disclosure. He also highlights the "disclosure gap" between what patients want and what health care professionals commonly provide after an error has occurred. |
