Risk factors associated with early breastfeeding cessation among first-time, low-income mothers.

Abstrakt: In the US, there has been progress in increasing breastfeeding (BF) initiation rates, but less improvement in BF duration and exclusivity. At higher risk for short BF duration are lower income mothers. Little is known regarding their early BF experience and its association with BF duration. Low-income, first time mothers from Sacramento, CA (N=180) are being followed from birth to 6 wk postpartum (pp). Data collection includes an in-person interview prior to hospital discharge (demographics; BF intentions, difficulties and practices; breast and nipple comfort; reasons for supplementation; and neonatal perception); and telephone interviews at day 4 (childbirth satisfaction; rooming-in practices; timing of milk onset; BF practices and difficulties; breast and nipple comfort; milk supply and infant latch concerns; and reasons for supplementation or weaning) and wk 6 pp (BF status, reasons for supplementation or weaning). Exposure to a doula care program is documented by a separate team. Obstetric data were extracted from the medical record. To date, 170 subjects have enrolled (35% Hispanic, 25% white, 20% African-American, and 20% mixed/other). Of those enrolled, 95% and 87% completed the day 4 and wk 6 interviews, respectively. Although 97% initiated BF, 72% were not exclusively BF at 6 wk pp, including 40% who weaned. Modifiable peri-natal factors associated with BF duration and exclusivity will be presented.
