Abstrakt: The article reports that computer programs and calculations used to determine drug doses can be incorrect. A case study of a three-month old infant in the intensive care unit is presented. Using a Microsoft Excel program and body weight to calculate doses of noradrenaline, the patient was mistakenly given a dose seven times higher than necessary for two hours. To prevent this problem from continuing, a paediatric resuscitation chart has been introduced for use in drug infusions. Efforts to reduce software and information technology related accidents are presented.
