Abstrakt: The Occupational Safety and Health Management System (OSHMS) is a protection system that has been designed for workers and construction services to ensure or avoid the risk of time, material, and work accidents that will be experienced by workers. For implementation of OSHMS in Indonesia is based on the Regulation of the Minister of Public Works and Public Housing of the Republic of Indonesia Number 10 of 2021 concerning Guidelines for Construction Safety Management Systems. In a real project still has so many problems. Therefore, the aim of this study is to identify high-risk levels for construction workers and the basic events caused by risk using fault tree analysis (FTA). The methods used in this study are field observation and data collection using questionnaires identified by expert judgments, the basic event cause of risk is identified by using FTA and will be given the solution. The results showed that 48 risks occurred in project. The kinds of works that have high risk were land clearing, formwork installation, pouring concrete, pouring concrete installation, steel column and beams work, roof work, and mechanical electrical installation. The impact of high-risk work is falling from height, electrical shock, struck and stabbed by material falls, and stricken pouring concrete. The basic event caused risk where workers were not careful, no punishment for workers, workers did not were personal protective equipment (PPE), uncomfortable feeling using PPE, and unavailability of PPE. The solution to solve the problem is supervision, training, and punishment implementation. [ABSTRACT FROM AUTHOR]
