A 12-year-old boy with recessive dystrophic epidermolysis bullosa (RDEB) presented with a non-healing ulcerated, verrucous plaque on his foot. The diagnosis was verruciform xanthoma, a rare benign neoplasm that typically manifests in the oral mucosa but can occur on extraoral sites. Treatment options include surgical excision, and awareness of this condition is crucial to differentiate it from more serious complications like squamous cell carcinoma.
