The article in the "Shoulder & Elbow" journal responds to a study comparing primary treatment of proximal humerus fractures with reverse shoulder arthroplasty (RSA) within 3 months of fracture to treatment provided more than 3 months after fracture. The study found better external rotation and forward flexion with no differences in complications or patient-reported outcome measures. However, the authors raise concerns about psychosocial factors, biomechanical stability, the potential for avoiding surgical procedures with non-operative treatment, and the impact of medical comorbidities on outcomes. They suggest that a randomized controlled trial is needed to fully evaluate the merits of different treatment strategies. [Extracted from the article] |
