Abstrakt:	The article discusses research which aimed to determine the proportion of older adults after care by a program modeled on the Naylor Transitional Care Model (TCM) return to receiving office-based care or instead transition to an alternative care setting. Cited are the observed 30-day readmission rate in the received services group versus the not receive services group and the effectiveness of the Home-Based Primary Care Program in helping participants remain at home and reduce healthcare costs.
