Abstrakt: A recent study conducted by the Minnesota Department of Health aimed to evaluate a cardiovascular care intervention that would increase access to comprehensive medication management (CMM) pharmacy care and improve vascular health goals among socially disadvantaged patients. The study found that CMM pharmacists were able to increase recruitment among patients in clinics serving diverse and socially vulnerable communities. Patients who partnered with CMM pharmacists were more likely to connect with system resources and achieve blood pressure and statin goals compared to those without CMM care. The study suggests that cultivating relationships between patients with social disadvantage and CMM pharmacists may improve health outcomes and connect patients to essential resources, potentially improving long-term cardiovascular outcomes.
