Factors influencing single agent methotrexate chemotherapy resistance in low risks gestational trophoblastic neoplasia in Indonesia single center trophoblast study.

Abstrakt: Objective: Gestational trophoblastic neoplasia (GTN) is a malignant entity of gestational trophoblastic disease. Singleagent methotrexate (MTX) is the treatment of choice in patients with stage I, II, and III GTN with International Federation of Gynecology and Obstetrics (FIGO) score <7 (low risk). Cases of chemoresistance to this single-agent chemotherapy have been reported, resulting in reduced remission rates. This study will assess which factors can influence single-agent MTX resistance in GTN. Methods: This study is an observational analytic study with a cross-sectional approach. Sampling was carried out using a total sampling technique using secondary data from the medical records of stage I, II, and III GTN patients with a FIGO score <7 who underwent single-agent chemotherapy MTX 50 mg from January 1, 2013-September 30, 2023, at single center trophoblast study, Indonesia. Patients were grouped into the chemoresistance or complete response group. Results: A total of 395 patients were included in this study. There were 305 cases of complete response (84.5%) and 90 cases of chemoresistance (15.5%). Bivariate analyses show that there was a significant difference between the result of chemotherapy to interval from index pregnancy (p=0.001), β-human chorionic gonadotropin (β-hCG) levels before treatment (p=0.001), pathological anatomy type (p=0.005), and FIGO stage (p=0.000). Multivariate analysis showed that β-hCG levels 1,000-10,000 before treatment (p=0.015) and stage >1 (p=0,000) were independent risk factors for MTX chemoresistance. Conclusion: Pretreatment β-hCG levels and FIGO stage were independent factors affecting single-agent MTX chemoresistance in stage I-III GTN patients with FIGO score <7. [ABSTRACT FROM AUTHOR]
