Intercostal neuralgia is a complex painful disorder characterized by intense, sharp-shooting, or burning pain, along the distribution of an intercostal nerve. It is a difficult-to-treat condition, in which pharmacological modalities of treatment often fail. It occurs around 40%-50% of patients commonly after thoracotomy. Intercostal neuralgia due to surgical injury of the intercostal nerve has traditionally been difficult to treat. There are several treatment options available, including systemic medications, topical or invasive nerve blocks, cryoablation, and radiofrequency ablation. We have successfully performed cryoneurolysis of intercostal nerves with the hybrid technique of combined ultrasound and fluoroscopic guidance for a 55-year-old male patient who had postthoracotomy status with immediate postprocedure pain relief without any complications. Cryoneurolysis has been demonstrated to be a safer intervention for the surrounding tissues, such as blood vessels and muscles. Visualizing the pleura during the procedure is the greatest benefit of using ultrasonography. [ABSTRACT FROM AUTHOR] |
