An 11-year-old girl was referred to a dermatology clinic for a rash on her legs and feet that appeared when she stood for longer periods and resolved when sitting or lying down. She also experienced leg numbness while sitting in vehicles. After extensive evaluation, the patient was diagnosed with BASCULE syndrome, a rare condition characterized by vasomotor dysfunction in the distal regions of the body. The syndrome presents with hypopigmented macules on a cyanotic background, and symptoms are relieved upon postural changes. Treatment options for BASCULE syndrome are limited, but H1 antihistamines may provide some relief. It is important to recognize this condition to avoid unnecessary diagnostic workup.
