A recent study conducted at the Royal Children's Hospital in Melbourne, Australia, examined the transition process for pediatric rheumatology patients at the Monash Children's Hospital. The study aimed to identify areas for improvement in the transition process. The research found that while the transition process was adequate, it could be enhanced through earlier discussions, improved referrals, and better documentation.
