This document is a systematic review and meta-analysis that examines the relationship between subclinical hypercortisolism, also known as mild autonomous cortisol secretion (MACS), and various comorbidities. The review finds that MACS is associated with an increased risk of comorbidities and that adrenalectomy (surgical removal of the adrenal gland) is more beneficial than conservative treatment for managing these comorbidities. However, the review acknowledges limitations in the definition and diagnostic criteria of MACS, variations in conservative treatment, and heterogeneity among studies.
