This review explores the potential of Indole-3-Carbinol (I3C) and 3,3-Diindolylmethane (DIM) as promising therapeutic agents for leukemia treatment and cancer chemoprevention. The biochemistry and mechanisms of action of I3C and DIM are examined, including their structures, properties, metabolism pathways, and molecular targets relevant to leukemia and cancer prevention. Preclinical studies investigating the effects of I3C and DIM on leukemia cell lines and animal models are summarized, highlighting key findings and implications for clinical translation. Additionally, clinical trials evaluating the safety, pharmacokinetics, and efficacy of I3C and DIM in leukemia treatment and cancer prevention are reviewed, along with ongoing and future clinical trials. Adverse effects, safety considerations and strategies for mitigating risks associated with I3C and DIM supplementation are discussed. Finally, future directions, challenges and unresolved questions in the field of I3C and DIM-based leukemia therapy and cancer chemoprevention are addressed, including potential synergies with existing treatments, exploration of combination therapies and areas for further research.
