Lane-Hamilton Syndrome: A Vasculitis Mimic Presenting with Diffuse Alveolar Haemorrhage and Positive ANCA Testing.

Abstrakt: Rheumatologists and pulmonologists have an important role in evaluating patients with diffuse alveolar haemorrhage (DAH). This is because different forms of systemic vasculitis and other systemic autoimmune disease may present with this life-threatening condition. In this article, we present a case of DAH in a 54-year-old male who tested positive for atypical perinuclear anti-neutrophilic cytoplasmic antibodies but was ultimately diagnosed with Lane-Hamilton syndrome (LHS), a coexistence of idiopathic pulmonary hemosiderosis (IPH) and coeliac disease (CD). The patient did not have known CD at his presentation. LHS has a relatively high mortality rate secondary to IPH. Despite the clinical severity of this entity and relative straightforward screening tests for CD, patients with this diagnosis continue to be missed. Treatment in the form of a gluten-free diet could improve overall outcomes in these patients when the diagnosis is made in a timely manner, highlighting the need to include CD in the initial differential diagnosis for DAH.
