This article discusses the prevalence, risk factors, and clinical outcomes of perforations following colorectal endoscopic submucosal dissection (ESD). The study analyzed data from 4025 procedures performed in French and Belgian centers between September 2019 and September 2022. The main finding was that conservative treatment was effective in 91.7% of perforations, with surgery required in only 0.8% of cases. Risk factors for perforation included larger lesion size, previous resection, distal colonic location, poor maneuverability, and severe fibrosis.
