Exploring the concordance between EUS-guided portal pressure gradient and the hepatic venous pressure gradient measurement. Time for a new gold standard?

Abstrakt: This article explores the concordance between Endoscopic Ultrasound (EUS)-guided portal pressure gradient (PPG) and the hepatic venous pressure gradient (HVPG) measurement in patients with established or suspected portal hypertension. The study found that EUS-PPG using a 22-gauge FNA needle is a reliable and safe technique for measuring the portal pressure gradient in cirrhotic patients. The correlation between EUS-PPG and HVPG was very good, suggesting that EUS-PPG could potentially become a new gold standard for estimating portal pressure gradient. However, there were a few cases with significant discrepancies between the two techniques, with the MASLD aetiology being associated with a higher occurrence of discrepancies.
