This document is a letter published in the journal Emergency Radiology. The authors discuss the diagnostic performance of spectral Doppler in acute appendicitis with an equivocal Alvarado score. They explain that the Alvarado score was used to enroll participants with suspected acute appendicitis, and they found that color flow in appendiceal ultrasound was observed in 84 out of 100 participants. The authors also mention the importance of incorporating criteria such as mean maximum outer diameter, peak systolic velocity, and resistive index to improve diagnostic accuracy. They outline an algorithm for the diagnostic workup of acute appendicitis, which includes initial assessment, ultrasonography, computed tomography, and alternative modalities. The authors suggest that artificial intelligence-based ultrasound diagnosis of acute appendicitis is an area worth investigating.
