Abstrakt: This article, published in the Arab Journal of Interventional Radiology, discusses the completeness and accuracy of preprocedure documentation for image-guided procedures at PHQ Hospital Gilgit. A retrospective review of 100 preprocedure documentation forms was conducted, and it was found that 95% of the forms were complete and accurate. The remaining 5% had incomplete or inaccurate information in areas such as informed consent, preoperative history, and medication. The study suggests that the lack of adherence to guidelines may be due to a lack of knowledge and inadequate supervision. Recommendations are made to improve training, establish a system for reviewing and correcting errors, and enforce adherence to guidelines. By addressing these issues, the quality of preprocedure documentation can be improved, leading to better patient care. [Extracted from the article]
